This four-part course includes: Part 1: Factors that Impact Fetal Health, Part 2: Intrapartum Risk Factors, Part 3: Impaired O2 Delivery to the Fetus, and Part 4: Neonatal Conditions. S.T.A.B.L.E. Foundations – Description and Module Objectives
In this four-part curriculum we will cover the following, Part 1 will define implicit bias and how biases and stereotypes affect decision-making in the clinical setting. Maternal conditions that may impact fetal and neonatal health are also discussed. Parts 2 and 3 will explore additional risk factors that can compromise the laboring patient and fetus or fetuses, and Part 4 will review neonatal conditions that require prompt recognition and stabilization. Please note: Completing this learning activity does not authorize you to add any skill discussed in the content to your practice if this function is not included in your current scope of practice.
After completing this learning activity, the participant should be able to:
Part 1: Factors that impact fetal health
- Recognize that implicit or unconscious bias can impact decision-making in the clinical setting.
- Identify the adverse health effects of obesity and diabetes for the pregnant patient, developing fetus, and neonate.
- Examine how hypertensive disorders during pregnancy can jeopardize maternal, fetal, and neonatal health.
Part 2: Intrapartum Risk Factors
- Classify the stages of labor and explore how fetal malposition or malpresentation can prolong labor.
- Identify two fetal malpresentations that increase the risk for cord prolapse.
- Differentiate between scalp swellings and signs of a subgaleal hemorrhage.
- Identify the differences between three types of twin gestation.
- Examine the impact of preterm birth on increased morbidity and mortality.
Part 3: Impaired Oxygen Delivery to the Fetus
- Identify how the following pregnancy complications can impair oxygen delivery to the fetus:
- Placental abruption
- Placenta previa
- Velamentous cord insertion / vasa previa
- Placenta accreta spectrum
- Uterine rupture
- Fetal-maternal transfusion
- Recognize how impaired oxygen delivery to the fetus can lead to hypoxic-ischemic encephalopathy (HIE).
- Identify candidates for therapeutic, neuroprotective hypothermia.
Part 4: Neonatal Conditions
- Summarize fetal circulation and the changes in circulation that occur after birth with transition to extrauterine life.
- Recognize the similarities between fetal circulation and persistent pulmonary hypertension of the newborn (PPHN).
- Identify the presentation of congenital diaphragmatic hernia (CDH) and tracheoesophageal fistula/esophageal atresia (TEF/EA) and the immediate stabilization of infants with CDH and TEF/EA.
- Recognize signs of bowel obstruction and how intestinal malrotation can evolve into midgut volvulus
- Differentiate between the three common types of spina bifida and exam findings that may be signs of spina bifida occulta.
